Service Hours Form
All NHS members are required to complete a minimum of 25 service hours per year. Service hours must be earned during each marking period of the year in order for you to remain current. Service hours may be submitted in one of two ways:
- If your group does not provide an official log of your service hours, you must complete a DPNHS Service Hours Form (file available on Canvas). Record the appropriate information, have it signed by your campus group sponsor or off-campus supervisor, then either upload an image of the signed form to the appropriate assignment in Canvas or return the signed form to Mrs. Wells. Complete a separate form for each campus group.
- If your group provides you with a log (like the hospital), take a picture of your log and upload it to the appropriate assignment in the NHS Canvas class. Your log must be signed by your supervisor.
Service hours earned with NHS will be submitted to guidance. Hours submitted to guidance are not automatically credited to NHS, so make sure you submit your hours at the end of each marking period.
